Nominations for the 2010 Physician of the Year

The Physician of the Year Award is the highest honor given to a member of the AzAFP. The award is decided through a nominating and interview process conducted by the AzAFP Awards Committee every January. The recipient is honored at an Awards Reception held during the annual AzAFP Academy Clinical Education Conference.

All AzAFP members are urged to consider their colleagues and submit nominations. The criteria for the AzAFP Family Physician of the Year award are as follows: a compassionate, caring family physician, community involvement that enhances the quality of life in his/her home area, and provides a credible role model as a healer and human being to his/her community, and as a professional in the science and art of medicine to colleagues, other health professionals, and especially to young physicians in training and medical students. The nominee must be in good standing in his/her medical community and be a member of AzAFP and AAFP.

A nomination application, along with supporting material (eg, letters of support), must be received in the AzAFP office by December 1, 2009. Nominations for the AAFP Exemplary Teaching Award

The awards acknowledge Academy members who deserve recognition of exemplary teaching skills as well as individuals who have implemented outstanding educational programs and/or developed innovative teaching models. This award is presented in two different categories:

  • full-time or part-time faculty
  • volunteer faculty

Selection Criteria and Nomination Requirements:

The nominees for the 2009 AAFP Exemplary Teaching Awards will be evaluated based upon the following criteria:
  1. Recognized for exemplary teaching skills and outstanding progression of abilities over several years by medical students, residents or peers; or
  2. Developed and implemented innovative curriculum, teaching model(s) or program(s) in a variety of educational spheres; and
  3. Must be a current member of the AAFP.
All nominations must include the following information to be eligible for consideration:
  1. Completed nomination form. (Please click here to download and complete the nomination form.)
  2. A typed copy of a current Curriculum Vitae.
  3. A minimum of three and a maximum of five letters of recommendation. At least two of the letters of recommendation must be submitted from individuals who are current or former students/residents who have been taught by the nominee.
  4. A recent head-and-shoulder photo (5×7 or larger is preferred) of the nominee must be submitted, or as a digital photo on a disk or electronically.
All supporting documentation should be photocopied on one side of 8 1/2 x 11 white copy paper.

Thomas W. Johnson Award for Career Contributions to Family Medicine Education

The Thomas W. Johnson Award for Career Contributions to Family Medicine Education is designed to be the highest honor presented by the Academy for outstanding contributions to Family Medicine education in undergraduate, graduate and continuing education spheres. It is intended to recognize a career of national prominence, reflecting leadership and long-term dedication to Family Medicine education rather than a single, significant contribution. Criteria and Nomination Requirements:

  • Nominee must be an AAFP member
  • Nominee is recognized for his/her lifetime contribution to medical education
  • Nominee has developed and implemented innovative curriculum, teaching model(s) or program(s) and engaged in local, state and national or international educational initiatives
  • Nominee is recognized for his/her participation and leadership in academic medical organizations
  • Nominee must have a national reputation
  • Nominee may not be a current AAFP staff member, though former AAFP staff members are eligible
Nominations must include the following information to be eligible for consideration:
  • A statement on why you believe the nominee should receive the Thomas W. Johnson Award for Career Contributions to Family Medicine Education must be submitted. The statement should include sufficient specific data to provide a working knowledge of nominee’s education efforts. Typed statement must not exceed one page in length. Please complete the nomination form for submission.
  • A typed copy of a current Curriculum Vitae, including data relating to AAFP membership and other medical organizations must be submitted.
  • A recent head-and-shoulders photo (5×7 or larger is preferred) of the nominee must be submitted in black and white or as a digital photo on a disk.
  • No more than 3 letters of endorsement may be added to the nomination package. Endorsement letters from only those individuals who have personally worked with the nominee in a teaching setting will be considered.
  • Nominees may be submitted only two consecutive years. Once a person has been nominated two consecutive years, he or she must wait two years before being re-nominated.
